608 Certification Card Replacement

If you currently hold a 608 Certification and you need a replacement card, please complete the form below so that we are able to verify your certification.

Once your certification has been verified, we'll provide you with instructions for purchasing a replacement card.

The cost of the card is $25.00. We will be sending replacement certification cards by USPS.
